Step By Step for Screening
Below you will find the step-by-step instructions for employees and faculty of Western Washington University (WWU) working with live laboratory animal specimens.
- Download the Vivarium Screening Form
- Sign and Date form before sending it in.
- Email to Kaiser Permanente On the Job (KPOJ) at occmedrn@kp.org (preferred) or Fax to (206) 877-0742 if email is unavailable.
- If pass, you are done!

- If screening does not pass, call (866) 967-9675 Option 2 or (877) 751-6214 to schedule a VIRTUAL appointment with a KPOJ provider.
- For virtual appointment, an Authorization Form should be filled out prior to the appointment. It does not need to be submitted, but the information contained in the form will be used during the appointment.
- When making the appointment, tell the Kaiser Permanente call center staff that you are from Western Washington University, Guarantor Number 4566033 for service. This information is also noted on the Authorization Form.
Frequently Asked Questions
First step is to download the form from the webpage and fill it in, making sure to sign and date it. Next, email the form to occmedrn@kp.org. A doctor will review the form and sign off on it if it passes the review. If not, there will be follow-up with a Kaiser Permanente Health Provider regarding the issue at a virtual appointment.
Any faculty or staff member at Western Washington University that will be handling live animals for care or research will need to complete the screening. The form will have to be completed every three years for faculty/staff that has direct, physical contact with live vertebrate** animals totaling 24 hours or more per year*.
*Year refers to a consecutive period of 365 days.
**live vertebrate animals does not include human beings.
Once the form is emailed to occmedrn@kp.org, the initial screening should take no more than 72 hours. Once the form has been reviewed, it will either be signed off on as passing, or not signed and issued as a fail, which will result in a longer process determined by how quickly the employee follows up with Kaiser Permanente.
The initial Vivarium Screening Form should be filled out fully, including signed and dated, then emailed to occmedrn@kp.org. If email is unavailable, the form can also be faxed to (206) 877-0742. Please note that email is the preferred way of sending in the form.
Should the initial screening fail due to health issues indicated by the employee/faculty member, then the employee will need to fill out the Authorization Form and call (866) 967-9675, Option 2 OR (877) 751-6214 to schedule a virtual appointment with Kaiser Permanente to go over the items on the questionnaire.
In case of a failed screening, the employee/faculty member will need to fill out the Authorization Form and call (866) 967-9675, Option 2 OR (877) 751-6214 to schedule a virtual appointment with Kaiser Permanente. When making this appointment, the person should let the scheduler know that they are from Western Washington University and our Guarantor number is 4566033 in order to receive services. This information is also included on the Authorization Form. The form does not need to be submitted to Kaiser Permanente, it only serves to make sure that the employee/faculty member has all the information necessary to get the appointment scheduled.
